<jats:p>Aorto-right atrial fistula (ARAF) is an uncommon but life-threatening complication of infective endocarditis (IE), often associated with extensive paravalvular destruction and heart failure. Owing to its rarity, optimal management remains challenging. We present a case of subacutre native IE complicated by ARAF, successfully treated by means of comprehensive surgical repair.</jats:p>
