Abstract

<jats:p>Overcrowding in reception departments and a shortage of specialized specialists require new organizational solutions. The introduction of the position of an emergency medical doctor in an inpatient emergency department in the Russian Federation is a promising area for optimizing the work of a multidisciplinary hospital. An analysis of the literature shows the existence of conceptual work on the organization of STOSM and modern clinical recommendations for the diagnosis of acute pancreatitis, however, there are no unified protocols that combine the organizational and clinical aspects of the work of an NSR doctor. The purpose: to develop and scientifically substantiate an organizational model for the work of an NSR physician in a STOSMP with a unified diagnostic algorithm for acute pancreatitis, including tactics for managing patients with an unconfirmed diagnosis and persistent abdominal pain syndrome. Materials and methods. The Russian and international sources were analyzed, as well as the clinical recommendations "Acute Pancreatitis" of the Russian Federation in 2024, the Chinese consensus in 2024, ACG 2024, IAP/APA 2025, JPN2021, NICE2026. Performed content analysis and comparative analysis of regulatory documents and literary sources; organizational modeling; the method of expert assessments (Delphic technique) with the participation of 15 experts. Results and discussion. An organizational model is presented that includes: functional responsibilities of the NSR doctor; a unified diagnostic algorithm for acute pancreatitis with step-by-step actions; differentiated protocols for triage zones (red/yellow/green); an algorithm for actions with an unconfirmed diagnosis; protocol for monitoring short-term stay beds and routing to a day hospital; severity assessment scales (SIRS, criteria for hypovolemia); medical record template; decision-making flowcharts. Findings. The developed model fills in the gaps in existing work by creating a specific working tool for the SMP doctor. The second part of the model (photochronometry of surgeons' workload) will be the subject of further research.</jats:p>
